COVID-19 Update

While the office has moved into Phase 2, we have elected to continue virtual services for the majority of our clients. When an office visit is deemed medically necessary between client and clinician, an in-person appointment will be scheduled. Any clients scheduled for an in-person appointment will be asked to sign a consent form. We have also created safety precautions for clients, staff and our office environment. You can find both the consent form and safety precautions below.

Our office staff can assist you easily via phone to answer questions, direct you to forms, and/or to schedule an appointment with your clinician. Office staff is available Monday – Friday, 8:30am – 4:30pm.
